Are you living with Alzheimer’s disease or another form of dementia?  Are you supporting someone who is?

It is often difficult for individuals and families to ask for help. The Alzheimer Society of New Brunswick's First Link® program can be the first step toward getting the kind of support you and your family need.

Connecting with First Link means you can:

  • Receive one-on-one or group support
  • Get early access to information, services, and support
  • Be referred to local healthcare providers and community services
  • Meet other people in similar circumstances and exchange experiences
  • Plan your future while you are still able to do so
  • Find resources and supports for caregivers
Who Qualifies?

The program can help if you or a family member are:

  • Diagnosed with Alzheimer’s disease or another form of dementia
  • Diagnosed with Mild Cognitive Impairment (MCI) or other memory problems
  • A caregiver for someone with dementia
  • Concerned you may have symptoms of dementia
How Do I Apply? (Process Steps)

To access free services and programs, you can reach out to First Link® staff directly.  A referral is not required.

Health professionals and community organizations play a key role in connecting those affected by dementia with the support they need. With your consent, they can submit a referral on your behalf. A First Link staff member will review your referral and contact you to follow up.

First Link® staff will discuss any concerns and help identify your needs. They will also connect you with local health services and the resources that best meet your unique circumstances. Staff will continue to support you during different stages of the disease.

What Do I Need? (List of Requirements)

If you are accessing First Link® through a referral, you will be asked to provide consent to share your information with the program.

 

 

If you are reaching out directly to the Alzheimer Society without a referral, you don’t need any documents.
