Do you need coverage for vision care for your four-year-old child?
Good vision is vital to your child’s overall health and development. All children should have annual eye exams by an optometrist starting at age three to help ensure normal vision. Healthy eyes are critical for success in school as 80% of school-age learning is visual.
The Vision Benefit for Four Year Olds covers one major eye exam and, if required, one set of corrective glasses to ensure all children receive proper vision care before starting school. The Benefit is an expansion of the Healthy Smiles, Clear Vision program. It is available for parents or guardians with or without vision insurance. It is administered by Medavie Blue Cross.
To be eligible for the Vision Benefit for Four Year Olds, your child must be:
- Four years old
- A resident of New Brunswick
Note: If you already have coverage under Healthy Smiles, Clear Vision, you can receive these benefits through that program.
Fill out the Vision Benefit for Four Year Olds application form. You can download the form by clicking this link or get a paper copy at your vision care provider’s office.
You will need to provide a copy of your child’s N.B. Medicare card and details on any health insurance coverage you may have with your application.

Note: Please submit the application form at least ten business days before your child’s vision appointment to give Medavie Blue Cross time to process your application. You will receive a benefit confirmation letter in the mail. Please bring this letter to your vision appointment. The identification number included with the letter is required for submitting claims to Medavie Blue Cross.
