This program covers feeding supplies and formulas, including:

  • Total Parenteral Nutrition (TPN) pump and supplies
  • Enteral feeding formula, pump rental and supplies
  • Dietary Supplement

This program does not cover:

  • Supplies not specifically related to the feeding

Please note:

  • Eligible services are paid monthly.
  • Quantities and frequencies are monitored and may be restricted.
  • Brand-name products are considered only when generics are not available or when generics don’t meet your medical needs. Justification will be required.
  • There is no cost to eligible clients for eligible therapeutic nutrients supplies. However, if you get services for which you are not eligible, you may need to reimburse the department for the amount paid on your behalf.

How Do I Apply Steps

Fill out the Client Information section of the form.

Take the form to an authorized prescriber. For the Dietary Supplement Application, this is a doctor, nurse practitioner, registered dietician or speech therapist. For the Enteral Feeding Application, it is a doctor, nurse practitioner or registered dietitian.

Your medical professional will complete the form and send it to a pharmacy or medical supplier on your behalf.

The pharmacy/supplier will send the application and cost estimate to Health Services for a decision.

Health Services will review the application. If your request is approved, the supplier and your case manager or social worker will let you know.

Once enteral feeding or dietary supplement is approved, you can pick up the approved amounts at the approved frequencies from the pharmacy/supplier who received the approval. They will bill Health Services directly.
