This program covers orthopedic items such as:

  • Specific custom-fitted braces and supports
  • Custom-made braces
  • Therapeutic and orthopedic design footwear. The client pays a participation fee of 10% of the cost of the footwear to a maximum of $20.    
  • Custom-made shoes and insoles
  • Modifications and repairs

This program does not cover:

  • Support bras
  • Cervical pillows
  • Soft or unfitted supports and braces
  • Non-custom insoles
  • Non-custom wrist braces and splints braces and supports for short-term use
  • Braces and supports for sports purposes

Please note: 

  • Most items are eligible once every two years for adults.
  • Modifications and repairs are paid as needed, but quantities and frequencies are monitored.

How Do I Apply Steps

Get a referral to an authorized orthopedic professional by a doctor or nurse practitioner for a specific orthopedic problem. Please see the Orthopedic Program Policy for a list of orthopedic professionals authorized for the various services.

Once you have a referral, make an appointment with an orthopedic professional. They will determine the device or product that best meets your needs. If the program benefit covers it, they will submit the application form and the prescription from your medical professional to Health Services.

If the program benefit covers it, they will submit the Orthose & Orthopedic Bracing Application Form or the Footwear and Orthotics Application Form along with the prescription from your medical professional to Health Services.

 

Health Services will review your application. If it is approved, Health Services will let the orthopedic professional know. They will contact you to provide the product or device. They will bill Health Services directly.
