How Do I Apply Steps

Make an appointment with your doctor, nurse practitioner or an enterostomal therapist. They will complete the Ostomy and Incontinence Application Form to prescribe the products you need.

The medical professional will give you the form to take to the pharmacy or medical supplier of your choice or will fax it to a supplier on your behalf.

The pharmacy/supplier will forward the application to Health Services.

Health Services will review your application. If approved, Health Services will give the supplier a specific list of the approved products, quantities, prices and billing frequencies. They will notify your case manager or social worker, if they have been involved in your request.

The supplier or your case manager or social worker will notify you of the decision.

The pharmacy or medical supplier will bill Health Services directly every month.
