Helps Social Assistance clients with specific medical conditions cover additional food costs for up to one year.

Who Qualifies?

This benefit is available to Social Development clients who:

  • are receiving palliative care
  • are HIV positive
  • have undergone a major organ transplant such as liver, kidney, heart or lung
  • are children 18 and under who are diabetic and insulin-dependent
  • have been diagnosed with AIDS, Celiac disease, Hepatitis C, Cystic Fibrosis, Chronic Inflammatory Bowel Disease (Ulcerative Colitis and Crohn's disease) or Phenylketonuria (PKU)
  • are cancer patients in active treatment
  • are on dialysis for chronic renal failure
How Do I Apply Steps

Have your physician complete the Request for Special Diet form, which must indicate the conditions from which your child suffers.

Send your completed Special Diet Form, along with the written estimate from your pharmacist to your case manager for further assessment.

Once your application is complete, we will review the information to see if you meet the program’s financial requirements. Renewals require a prescription from your doctor or a note from your nurse practitioner, dietitian, or nutritionist requesting the benefit continue, and indicating that your child’s medical condition has not changed.
