Are you an older person looking for help to keep your feet and nails healthy?

If you are, Income Assistance may be able to help you access footcare services. The support is provided to help individuals maintain good foot health and remain mobile, healthy and independent.

 

Who Qualifies?

  • You must be receiving Income Assistance, and
  • Have a need for footcare services.

 

What do I need?

To apply, you can reach out to your case manager to guide you through the process and determine which form to complete. 

  • You must complete and send in the Non-Diabetic Foot/Nail Care form, found here
  • If you have diabetes, you must complete and send in the Diabetic Foot/Nail Care form, found here

 

How Do I Apply Steps

To apply, you can first speak with your case manager to access the form you need to fill out. There is the Non-Diabetic Foot-Nail Care form for seniors who don’t have diabetes and a Diabetic Foot-Nail Care form for people with diabetes.

For both forms, you must complete the “Client Information Section.” You have to provide your address and phone number and date of birth.

Your physician or nurse practitioner must fill out Section 1 and Parts A, B and C of the form and return it to you.

After the form is completed, you can mail it to:

An administrator from Income Assistance will advise your case manager if the application has been approved. If approved, your case manager will work with you to find a qualified footcare service provider in your region. The service is reviewed annually to determine whether it’s still necessary.
